ENDOSCOPIC TREATMENT OF BLEEDING OF THE UPPERGASTROINTESTINAL TRACT.LITERARY REVIEW.

Abstract

Relevance. Acute gastrointestinal bleeding remains a pressing problem in emergency abdominal surgery. In most cases,
gastrointestinal bleeding stops spontaneously. In other cases, one intervention or another is required. The choice of
treatment depends on the source of the bleeding. Endoscopic procedures are effective methods of stopping bleeding,
allowing one to avoid extensive laparotomy surgery. The surgeon must have a choice between different methods of
hemostasis based on specific operating conditions.
Purpose. Analysis of problems regarding the treatment of upper gastrointestinal bleeding, the timeliness of prescribing
drug and endoscopic hemostatic therapy according to modern literature
Search strategy. The study examined full-text publications in English and Russian, which are devoted to the endoscopic
treatment of gastrointestinal bleeding from the upper digestive tract in foreign countries. The following search engines were
used in the literature search process: Pubmed, Web of science, Cyberleninka, Google Scholar using keywords. Publications
from 2009 to 2022 were studied and analyzed; the articles were mainly foreign and in English. More than 130 publications
were identified on this topic. Of these, 86 publications corresponded to the purpose of our study. Inclusion criteria:
Publications of level of evidence A, B: meta-analyses, systematic reviews, cohort and cross-sectional studies. Exclusion
criteria: brief reports, newspaper articles and personal communications.
Results and conclusions. The literature review presents issues of endoscopic treatment of gastrointestinal bleeding
from the upper digestive tract. The choice, strategy and techniques are substantiated in various clinical situations, including
recurrent bleeding. It tells about hemostatic devices and drugs, about the advantages of different types of clips used for the
purpose of hemostasis.
